Overall wellness examination to establish an ongoing primary care- patient relationship that is centered around the integration of primary, secondary and tertiary care to achieve an overall daily health maintenance. 

Primary care visit may consist of some of the following services:

Physical assessment, blood work, EKG, order of primary screenings depending on age/risk factors such as referrals for colonoscopy, mammograms etc.. Referrals for specialists may also be integrated into the care plan as well.

Primary Care Management examples but are not limited to the following:

• Hypertension Management

• Diabetes Management

• Asthma Management

• GI issues

• Mental Health such as Anxiety, Depression etc..

• Weight loss Management
